Forms of application for the License, License form, application for modification of premises, License for premises modification, application for renewal of license, for replacement license and application for transfer of license shall be made pursuant to the attachments hereto, as follows: (1) Application for the License to Conduct Controlled Operations under Category 3, use: Tor Por Nor 3 Form (2) Application to Conduct Controlled Operations under Category 3, use Tor Por Nor 4 Form (3) Application for Premises Modification, use Tor Por Nor 4 Form and write or stamp these words “MODIFIED” in red ink on the top part of the License. (4) Application for Modification of premises conducting controlled operations under Category 3, use: Tor Por Nor 5 Form (5) Application to renew the License to Conduct Controlled Operations under Category 3, use: Tor Por Nor 6 Form (6) Application for Replacement License to Conduct Controlled Operations under Category 3, use: Tor Por Nor 7 Form (7) Application to Transfer the License to Conduct Controlled Operations under Category 3, use: Tor Por Nor 8 Form.

NOTIFICATION OF THE DEPARTMENT OF

ENERGY BUSINESSES

Re: Determination of Forms of Application for License, Form of License, Application for Modification of Premises, Form of License for Modification, Form of Application for License Renewal, Replacement License and Application for Transfer of License governing the Conduct of Controlled Operations under Category 3,

B.E. 2546

By virtue of Sections 25, para one, 27 para one, 28, 32 para one, 33 para one and 34 para one of the Ministerial Regulations determining the criteria, procedure and terms governing notification, license and rates of fees pertaining to the conduct of fuel oil operations, B.E. 2546, issued pursuant to the Fuel Oils Control Act, B.E. 2542, the Director-General of the Department of Energy Businesses hereby issue this Notification, as follows:

1. This Notification will be called: “Notification of the Department of Energy Businesses re: Determination of Forms of Application for License, Form of License, Application for Modification of Premises, Form of Application for License Renewal, Replacement License and Application for Transfer of License governing the Conduct of Controlled Operations under Category 3, B.E. 2546”.

2. This Notification shall come in to force as from today onwards.

(1) Application for the License to Conduct Controlled Operations under Category 3, use: Tor Por Nor 3 Form

(2) Application to Conduct Controlled Operations under Category 3, use Tor Por Nor 4 Form

(3) Application for Premises Modification, use Tor Por Nor 4 Form and write or stamp these words “MODIFIED” in red ink on the top part of the License.

(4) Application for Modification of premises conducting controlled operations under Category 3, use: Tor Por Nor 5 Form

(5) Application to renew the License to Conduct Controlled Operations under Category 3, use: Tor Por Nor 6 Form

(6) Application for Replacement License to Conduct Controlled Operations under Category 3, use: Tor Por Nor 7 Form

(7) Application to Transfer the License to Conduct Controlled Operations under Category 3, use: Tor Por Nor 8 Form
